How to Register

This expanded access program (EAP) is open to all treatment centers in the U.S. If you would like to become a participating site, you will need to register your treatment center.

The registration process: From inquiry to enrollment

Below is an overview of the steps to register your site and begin enrolling patients. The EAP Coordinating Center will help registrants through the documentation and approval processes. Monitoring of this program is anticipated to be handled remotely, unless a need develops for an onsite visit to your center.
